A MUM-of-two wrote a heartbreaking final post to announce her own tragic death after passing away from cancer.

Kate Rackham, who was diagnosed with incurable secondary breast cancer at the age of 39, tweeted “If you’re reading this, I’ve died”, shortly after losing her battle.

The mum-of-two, from Manchester, wrote the post and held it in drafts on her account named ‘Teacher with Cancer‘ before sadly passing away.

In the emotional tweet posted on Thursday, she said: “If you’re reading this, it means I have died.
